This form is for Class of 2019 Invitees to sign on as Angelworks Senior Models!  Please choose your payment amount, and fill out the information requested to complete your signup.  After these two have been completed, you are officially signed on as an Angelworks Class of 2019 Senior Model!

Your signup below, and your payment constitutes acceptance of the Class of 2019 Model Contract as stated below:

MODEL:  I understand all of the details of the Angelworks 2019 Senior Model Program (also referred to as our Class of 2019 Senior Angel Squad).  I understand my part in the program, and agree to abide by all policies of the program.  I agree and commit to have my Senior Portraits taken by Charity at Angelworks prior to my graduation in 2019, and to represent ONLY Angelworks from now until the end of the 2018-19 school year..  I also agree that while representing Angelworks (which begins on the date this contract is submitted and ends on the date of my graduation in 2019), I will not USE or REPRESENT any other photographer, photography student, or photography studio (including any person of any age working to build a photography portfolio).  Exceptions will be made for school and other organizations that require sports, activity, and function photographs be taken by a contract photographer/studio.  Angelworks has the right to use any of my photographs/images for all marketing/advertising/ and display purposes.  Violation of any portion of this contract will result in the contract becoming null and void, and all benefits received as an Angelworks 2019 Senior Model (sessions, images, video and other), will be due at their full face value.

PARENT:  As the parent/gaurdian of the model named above, I understand a agree to the policies of the Angelworks 2019 Senior Model Program.  I have also read and agree to the information above (as agreed to by my child), and grant permission for my daughter/son to participate in the Angelworks 2019 Senior Model Program.
